Spedforms Signature Page - not able to edit or not appearing

Users not able to edit Signature page.

Does not have either Initial or Re-evaluation selected at the top of the Evaluation Report list of sections. In order for the signature page to show at the bottom, one of the two options must be checked and the page saved.
